Community Persistent Pain Management Service
The PCT cluster is seeking expressions of interest from suitably qualified and experienced organisations to provide a community based persistent (chronic) pain service. This service will replace the current acute hospital based services and will provide psychosocial and physical assessment and management of persistent (chronic) pain.
Core services: The provider should be able to demonstrate direct access to:
• Specialist Physiotherapists
• Specialist Nurses
• Specialist medical interventions and advice
• Psychological Therapies
• Occupational therapy
The service will benefit patients by delivering a more efficient and effective care pathway closer to home.
Services will be based in a number of community based locations across the PCT allowing for convenient access.
Bidders will be expected to demonstrate how seamless integration of the patient pathway would be achieved without duplication and a seamless transition of patients currently within the acute service providers.
